Scheduling Appointments

To schedule an appointment, please call our office during business hours.  You will be asked about the nature of your visit in order to book the most appropriate appointment for you.

Canceling Appointments

If you need to cancel or reschedule your appointment, please call us as soon as possible.  This will allow another patient to use your appointment time.  We require a 24 hour notice to cancel or reschedule an appointment.  If 24 hour notice is not given, you will be charged a $50 cancellation fee for an established patient, or a $100 fee for a new patient appointment. 

Late Appointments

We respectfully ask you to arrive 15 minutes prior to the time of your scheduled appointment to check in.  This allows time for us to copy your insurance card, have you update paperwork and use the restroom if needed.  Your scheduled appointment time is the time we expect Dr. Trebian to be entering the exam room to see you.  We respect the time of our patients and we strive to stay on schedule so that you don't have to wait long.  If you arrive late for your appointment we will try to fit you in as best we can.  However, you may be asked to wait or if you are more than 15 minutes late, you may be asked to reschedule your appointment. 

Emergencies

If you are experiencing a serious or life-threatening emergency dial 911.

For our patients who are experiencing urgent medical problems, same day or next day appointments are usually available.  For medical emergencies outside of office hours, a health care provider is on call at all times and can be reached by calling our main office number and following the instructions to have the doctor paged.

Please do not call the emergency line if you are not having an urgent medical problem and you can wait until normal business hours.

We do not refill prescriptions or birth control pills via the emergency line.

Medical Records

You are entitled to copies of your medical records, whether for yourself or another medical provider.  You will need to sign a medical records release authorization (located HERE), which you may fax, mail, or drop off to our office.  Our office will then provide a copy of your records in a timely manner.  We do not charge a fee for medical records to be faxed to another healthcare provider.  We do charge a prepayment fee of $1 per page plus the cost of postage if we are asked to copy the records. 

Prescriptions

Prescription refills are processed only during our normal office hours.  Please plan accordingly and do not wait until the last minute to request a refill.  If you would like to order your prescription through a mail order pharmacy, you can obtain a mail order form from your insurance provider’s website.  Mail or fax the completed form, along with the written prescription from your practitioner directly to your mail order pharmacy.

Payments

Co-payments are due at time of service.  As a courtesy to you we will submit a claim to your insurance company for the charges you incur.  Please make sure to show your insurance card at every appointment. After the insurance processes your claim, you will receive a statement from us and any remaining balance is due within 10 days of the statement date.  Late payments will incur additional fees.  If you do not provide current insurance information and your claim is denied, we may refuse to resubmit the claim and you will owe the full amount charged for the visit. 

Pregnancy Disability Forms

Check with your Human Resources Department regarding what type and amount of maternity leave your company offers.  Some employers allow maternity disability to start two weeks before the due date while others expect women to work until their due date.  Standard postpartum leave is six weeks following a vaginal birth and eight weeks after a cesarean delivery.  If your employer has more than 50 employees, they are obligated by Federal law to offer you additional unpaid parental leave for another three months.

Forms to apply for pregnancy disability leave are available through your human resources department.  To apply for disability leave, please complete the patient sections of your forms and bring them to our office along with explicit instructions on where they are to be sent.  Please include a stamped envelope if the forms are to be mailed.  You must also complete a medical records release form authorizing us to release your medical information.  We will complete one disability form per pregnancy.  All other requests may incur a $25 fee per form.

If you do not see your insurance listed please call the office to check network status. We apologize for any incorrect information listed on insurance company websites or provided by insurance company customer service. While we have tried to have many insurance companies correct our participation status, we do not have control over their website content or provider directories. The information listed below is accurate. We no longer submit any out of network claims. We apologize for any inconvenience.

We are in-network for the following companies and will submit a claim on your behalf:

Aetna (all employer plans except Premier Care Network), Alliance, Anthem BCBS (all employer plans except Blue Priority WI), Anthem BCBS Pathway Network (through Healthcare Exchange), Centivo, Chorus (formerly known as Together w/CCHP), Cigna, HealthEOS, Health Payment Systems (HPS), Humana, Multiplan, Network Health Plan (employer plans and those offered through the Healthcare Exchange), PHCS, Smart Health, Trilogy, United Healthcare, WPS (PPO/Statewide).

We are out of network for the following companies and will NOT submit a claim: 

Aetna Premier Care Network, Anthem BCBS Blue Priority WI,  Anthem BCBS individual plans, Aurora employer plans, Dean Health Plan, First Health, Health Partners, Medical College of Wisconsin employer plans, Prevea, WPS Arise, and WPS Select. We are out-of-network for all plans offered through the Healthcare Exchange except: Anthem BCBS Pathway Network, Network Health Plan, and Chorus.

We do not accept or schedule appointments for any patients who have: 

Badgercare, Medicare, Tricare, or any Risk Sharing Plans
